How to apply

The process for making a claim is different depending on whether you are a worker or you are a dependant of a worker.

Lodging an application with us is free and we need to receive your application in your lifetime. Unfortunately, we are unable to assess applications that have been received after a worker or their dependent has passed away.

Application process for workers

Before lodging an application you must be examined by a doctor and be found to have (or be suspected to have) a dust disease covered by the Scheme.

You need to fill out an application form and lodge it with us.

You should include:

  • a doctor's report stating that you have (or are suspected to have) a compensable dust disease
  • documents proving your identity and of anyone claimed as dependants
  • any medical information showing that a dust disease exists, (eg respiratory tests, x-rays taken within the last 6 months).

Application process for dependants

You need to fill out an application form and lodge it with us.

You should include:

  • documents proving your identity and of any others claimed as dependants
  • documents proving that you were dependant for support on the deceased worker (eg marriage certificate, birth certificate, etc)
  • documents showing your financial circumstances at the date of the worker's death (eg Centrelink/Dept of Veteran Affairs statements, payslips, income tax returns etc).

If the deceased worker was not receiving benefits from us in their lifetime, you will need to complete a supplementary application form.

You must supply us with two forms of ID when you apply.

Medical assessments

If you don't know if you have a dust disease, we can arrange a free medical assessment. These can include:

  • x-rays of your lungs
  • a lung function testing
  • a medical consultation
